Ledger Line POS / Telemedicine and pharmacy

Short answerRead this first

How does an online pharmacy get a merchant account?

Hold state pharmacy licenses for every state you ship to, obtain certification from a recognized body such as LegitScript or NABP, and show prescription verification and privacy controls. Telemedicine merchant account applicants also show provider licenses for each patient state.

The right setup for telehealth merchants

What fits

A healthcare checkout that ties each charge to a prescriber visit or a licensed pharmacy order.

Why it matters here

Pharmacy and telehealth sit in the strictest card network tier, and certification often shortens review.

Most common setup mistakes

  • Selling prescription products without a licensed prescriber step
  • No LegitScript or similar certification
  • Mixing consultations and medication on one unclear charge

Send us one statement and Dewey will tell you if your setup fits your telehealth business.

  • Step 2

    Optimized POS and payment setup

We set up checkout for visits, memberships and prescriptions so each charge is clear to the patient and to underwriting. In person clinics get counter terminals set up alongside online billing.

  • Step 3

No long term contracts. No termination fees.

Telehealth rules shift fast, and you should be able to adjust without penalties. We place accounts with no long term contracts and no early termination fees.

  • Step 4

    Keep more of every swipe

Telehealth statements often include high risk pricing, registration costs and membership billing fees. We show you which charges are required by the category and which are not.

Why pharmacies are Tier 1

Visa lists card absent sales of prescription drugs under MCC 5122 and 5912 as Tier 1 in its integrity risk program, because illegal sales could seriously harm people. Mastercard also requires registration for non face to face pharmaceutical sales under those codes.

Visa notes an exemption from the registration step for merchants accredited by the National Association of Boards of Pharmacy programs or certified by LegitScript.

LegitScript certification

LegitScript states that Visa and Mastercard require pharmacies and telehealth providers that process card not present transactions to be certified by a recognized authority, and that LegitScript is one of them. Its healthcare certification covers online and retail pharmacies, telehealth providers that facilitate prescriptions, and related supply chain businesses.

Certification reviews licensing, prescribing practices and site content, so it doubles as a ready made underwriting package for a telehealth merchant account.

Online pharmacy merchant account applications that already hold certification often move through review with fewer document requests, because much of the licensing and prescribing review has been completed by the certifying body.

GLP 1 and compounded drug scrutiny

The FDA has published concerns about unapproved GLP 1 drugs used for weight loss, including compounded and counterfeit versions. Weight loss telehealth programs are among the most closely reviewed files today, and acquirers ask where the medication comes from.

Name your dispensing pharmacy and its licenses, and keep marketing claims aligned with the prescribing process.

What strengthens online pharmacy payment processing

Bring pharmacy and provider licenses, your certification status, sample patient intake, refund policy and the subscription terms if you bill memberships. Show HIPAA privacy practices and how prescriptions are verified before dispensing.

Cash pay clinics and med spas offering telehealth visits should separate service fees from medication charges so each is clearly described.

Separating visits, memberships and medication

Telehealth businesses often bill a visit fee, a monthly membership and medication. Showing each as a distinct line, with its own terms, helps underwriters understand what is being sold and helps patients recognize charges. Memberships that renew automatically also fall under Visa's 2020 subscription rules.

If a partner pharmacy dispenses, explain who charges the patient for medication and where the pharmacy is licensed.

Privacy and records

Payment flows should collect only what billing requires, keeping clinical details in your HIPAA protected systems. Keep prescribing records, licenses and certification renewals organized so you can answer acquirer reviews quickly.

Licensing across state lines

Telehealth providers generally must be licensed in the state where the patient is located, and pharmacies must hold licenses or permits for the states they ship to. Build a grid that shows each state you serve, the licensed providers and the dispensing pharmacy, and keep renewal dates on it.

That grid is often the first document an underwriter asks for in a telemedicine merchant account review, and it is also part of what LegitScript checks during certification.

Update it whenever you add a state, a provider or a pharmacy partner, and share the update with your acquirer before launching there.

How we place pharmacy merchant account payment gateway files

We package licenses and certification with your payment flow and place it with acquirers that register healthcare merchants. Reserve terms are set by the acquirer after review, in writing.

Clinics that are still working on certification should tell us where they are in the process. Knowing the timeline helps plan the application. Pharmacy merchant account files that arrive with licenses, certification and a clear medication sourcing story are the ones reviewers can move forward without a long list of follow up questions, which saves weeks for a clinic eager to launch or expand into new states.

Dewey Kopenga, in payment processing since 2018, reviews every file himself before it goes to underwriting. Send a statement for a free review.

What to check on your telehealth statement

We check these and every other line during a merchant statement analysis. New to statements? Read how to read a merchant statement.

  • Registration fees passed through more than once
  • High risk rates on routine visit payments
  • Disputes from memberships customers forgot
  • Gateway fees charged on refunds

Common questions

Still not sure? Ask us directly.

Do telehealth companies need LegitScript?

LegitScript says Visa and Mastercard require card not present pharmacies and telehealth providers that facilitate prescriptions to be certified by a recognized authority.

Is a retail pharmacy counter high risk?

Visa's Tier 1 designation applies to card absent sales; walk in counter sales are reviewed differently.

Why do GLP 1 programs face more review?

The FDA has warned about unapproved, compounded and counterfeit GLP 1 drugs, so acquirers check sourcing closely.

Does NABP accreditation help?

Yes. Visa exempts NABP accredited pharmacies from its separate registration step.

What is a telehealth payment processing file?

Licenses for every patient state, certification status, intake and prescribing workflow, refund policy and subscription terms if you bill memberships.
